Production Forms: Are you already a member and need to start or continue your production? Just choose a form below, download or print, fill it out and send it in! You will need the free Adobe Acrobat Reader program installed. You can get it here.

  • Project Proposal Form- Just fill this out and FAX or hand it in. This is where you begin your production's journey!

    PDF- You can download and print this form.

  • Equipment Reservation Form - If your project is approved to use field equipment, you will need to fill out one of these to reserve it.

    Web Form - You can also fill this form out online. Remember that the reservation will not be valid until you receive confirmation!

  • Facility Reservation Form - If your project is approved to use an in-house facility, you will need to fill out this form to reserve it.

    PDF- You can download, print this form and submit it in person or fax it to us.

  • Individual Release Form- You should have one of these forms for each person appearing as talent.

    PDF- You keep a copy of this for your records.

  • Playback Request Form- When you are finished with your production, this is what you need to have it put on the air.

    PDF- You must be a CMC member to use this form for submitting programs.

  • Bulletin Board Request Form - Announcements for Organizations or Local Residents with Lost Pets.

  • Storyboard Form- Map out your production using this blank storyboard.

  • Site Check Form- Use this form check out your field site before you start shooting.

  • Video Tape Log- After your done shooting, use this form to log your footage.

    PDF- You can download and print this form.

  • Edit Decision List- Keep track of all of your edits from your final project.

  • Music Cue Sheet- Keep track of all of your music.
