- Project Proposal Form- Just fill this out and FAX or hand it in. This is where you begin your production's journey!

- Equipment Reservation Form - If your project is approved to use field equipment, you will need to fill out one of these to reserve it.

- Facility Reservation Form - If your project is approved to use an in-house facility, you will need to fill out this form to reserve it.

- Individual Release Form- You should have one of these forms for each person appearing as talent.
PDF -You keep a copy of this for your records.
- Playback Request Form- When you are finished with your production, this is what you need to have it put on the air.
